Members of national junior squads, who are not members of national junior teams, should not be identified for entry on the Drug Free Sport NZ register.

What athlete contact information is required?

The Athlete Consent and details forms must be completed in full for an athlete to be added to the out of competition testing Register. The information required on these forms includes:
  • Full name
  • Sport
  • Discipline (ie, the athlete's position in a team, weight category or specialist event)
  • Date of birth
  • Address
  • Usual address
  • Alternate address
  • Overseas address where relevant
  • Contact telephone numbers
  • Contact e-mail address
  • Ranking in sport
  • Team, squad, disability (if appropriate)
  • Training information (days, times and usual venues with the addresses)
  • Coach information
Please Note
Athletes will be required to provide identification at the testing venue.

Amending existing athlete details.

Please check that existing athlete contact details are correct and complete. If any amendments are required, please add these new details to the form for the applicable athlete and return the form to Drug Free Sport NZ.

Adding new athletes to the register?

Please check that all athletes in your sport, who are competing at the appropriate national and international level, are on your Register (to obtain a current copy of your Register please contact the Drug Free Sport NZ). If not, please send athletes to be added the Consent and Details Forms on your letterhead for completion. These forms must be accompanied by the following information to be retained by the athlete :
  • Wallet Guide
  • Drug Testing - Your Rights & Responsibilities pamphlet
  • Asthma Guidelines sheet
  • Nutritional Supplements pamphlet.
Please order the amount of information required by contacting Drug Free Sport NZ by email or order from this site.

You may also wish to include a copy of your Sport's anti-doping policy/rules.

The athletes must return the completed forms to their National Sporting Organisation. On receipt of the forms, please check that all information has been provided by the athlete. If information is missing, please return the form to the athlete for full completion.

Sports may then copy the forms for their own records (if desired) and forward completed forms (originals or photocopies) to the Drug Free Sport NZ. If all the required information is on the Forms the athlete will then be entered on to the OOC Register for your sport and be subject to OOC testing until you advise us to remove the athlete (see "Deleting Athletes From The Register" below).

Deleting athletes from the register.

If an athlete is no longer competing at a level where s/he should be on the OOC register, or has retired, his/her name should be removed from the register. Drug Free Sport NZ requires written advice from the NSO before the athlete can be removed from the OOC testing register

(Leaving the athlete's name off a new list is not sufficient and the athlete will remain on the register.)

To remove an athlete from the register when updating your list, draw a line through the athlete's name and print the word "remove" next to it.

Example
Tom Black 10 Blue Street Phone 333333 REMOVE

Retirement

If an athlete retires, Drug Free Sport NZ should be immediately advised in writing, by email or by using the attached proforma (see below for link to a word doc) that can be filled out and faxed to the Agency as soon as this information becomes available. Failure to do this may result in the athlete continuing to be selected for OOC testing. In such a case there is the possibility that the athlete may refuse to undertake the test, which may result in the athlete receiving a similar sanction to that of a positive test result.
