Consent and Details Forms

National Sporting Organisations work together with Drug Free Sport NZ to build a list of athletes to be added to the Out of competition (OOC) drug testing register.

For an athlete to be added they need to provide us with Consent and Details Forms completed in full. The Consent Form is required so we have written documentation to say that athletes have read and understand their rights & responsibilities with regards to drug testing. The Details form provides us with specific information regarding an athlete's whereabouts so an athlete can be tested anytime, anywhere once they are added to the drug testing register.

Model Consent and Details Forms must be amended by each sport to show the national sporting organisation and international federation names where indicated,  as well as the return person and address at your Organisation on the bottom of the Details sheet. The Consent & Details forms should then be printed on to the letterhead of the sport and sent to the athlete for completion. The consent & details forms must be accompanied by the following educational material for the athlete when they are sent out for completion:

• Drug Free Sport NZ Wallet Guide.
• The Rights & Responsibilities Pamphlet.
• Athlete Handbook.

When the completed forms are received back at your Organisation you should:

• Check the forms to see that all details have been completed correctly and return any incomplete forms to the athlete(s) for completion
• Photocopy forms for your records (if desired)
• Send the completed forms to the Drug Free Sport NZ.
